Please fax (888-401-2979) or snail mail the following documentation
to us, and we will get you all the information you need to place
your first order:
Copy
of your tax ID or resale certificate in the store's name, PLUS Copies
of any TWO of the following:

Yellow pages ad or listing for the store |
Photograph
of outside of store
(including signage with store name) |
Title
page to the lease for store location
(dollar amounts may be crossed out) |
| Gas
or electric bill in the store's name and address |
Please
note that we must have the above documents on file before we can
process your wholesale catalog request. These requirements are not
meant to inconvenience you; rather, they have been established to
protect your interests as a store owner, and we appreciate your
understanding. If you have any questions, feel free to contact us
via e-mail.
In addition, there is a $7 fee for wholesale catalogs sent to Canada
or a $9 fee for wholesale catalogs sent overseas, which is fully
refundable upon request with your first qualifying wholesale order.
